Costal cartilage grafted aｔ this clinic [1/February/2019]

人に咬まれて耳介外側の欠損となり、
すでに他院で2回も手術を行われたが
耳介外側の欠損症となっている。
しかもTPFは既に使用されていた。
当院で2月1日肋軟骨移植術を行った。

Auricle is reconstructed.
耳介が再建されている。
The outline for the 2nd-stage operation.
耳立て手術のデザイン。
Reconstructed auricle is detouched.
Ultra-delicate split thickness scalp skin
[UDSTS] is harvested.
Skin is graftted
on posterior auriclar portion.
耳介後面に頭皮分層皮膚を移植した。
Minor repair done in Helix portion.
耳輪の一部を切除して修正した。
At the end of the 2nd-stage operation
with tie-over method.
タイオーバーをかけて植皮に軽い圧迫を加え手術終了。

Harvested site with Bolster fixation sutures.
耳甲介部より耳介軟骨を採取した。
採取部には、ボルスター縫合固定を行った。

under the ala skin pocket.
鼻翼皮下ポケット下に耳介軟骨を移植して縫合した。

1st-stage operation for lobule type microtia

耳垂残存型小耳症の肋軟骨移植術
Preoperative appearance
of lobule type microtia.
Determine the proper anatomocal location
with transparent film template.
The outline for the 1st-stage operation.
Fabricated 3-dimensional costal cartilage frame
[3-D frame] and paper template.
Skin flaps formed,
anterior skin flap of the lobule,
posterior skin flap of the lobule,
mastiod skin flap,
tragus skin flap.
All remnant ear cartilage removed.
Skin pocket created.
3-D frame is grafted
under the skin pocket
and reconstructed auricle.
At the end of the 1st-stage operation
with Bolster fixation sutures.

小耳症手術による合併症
一過性の顔面神経麻痺 浅側頭動・静脈の血行不良による植皮の生着不良 感染 移植軟骨の露出 気胸 術後肺炎 縫合不全 ハゲ 床ずれ その他
上記のような合併症が生じた場合は、症状に応じて対処致します。
場合によっては再手術を行う可能性もあります。

耳垂残存型小耳症の肋軟骨移植術
Preoperative appearance
of lobule type microtia.
Determine the proper anatomocal location
with transparent film template.
The outline for the 1st-stage operation.
Fabricated 3-dimensional costal cartilage frame
[3-D frame] and paper template.
Skin flaps formed,
anterior skin flap of the lobule,
posterior skin flap of the lobule,
mastiod skin flap,
tragus skin flap.
All remnant ear cartilage removed.
Skin pocket created.
3-D frame is grafted
under the skin pocket
and reconstructed auricle.
At the end of the 1st-stage operation
with Bolster fixation sutures.
