1st-stage operation fot atypical small concha type microtia

非定型的な小耳甲介型小耳症の肋軟骨移植術



Preoperative appearance of the small concha type microtia.
There are 2 fistulas in front of the lobure.

特殊な小耳甲介型小耳症の術前。
耳垂前方に相当する部分に耳婁孔が2個存在する。



The outline for the reconstruction of the auricle.

手術デザイン。



Lobule splited and skin flaps formed.
[anterior skin flap of the lobule,
posterior skin flap of the lobule,
tragus skin flap,
mastoid skin flap]
And skin pocket created.

4枚の皮弁形成を行い、皮下ポケットを作成した。



Excised fistula and remnant ear cartilage.

摘出した遺残耳介軟骨及び婁孔入口部。



There is wide skin inside the fistula.

婁孔の入口部を開くと広い皮膚の空間が存在した。



Fabricated 3 dimensional costal cartilage frame [3-D frame]
　and paper template.
　


3-D frame is grafted under the skin pocket.

３次元肋軟骨フレームを皮下ポケットに移植して再建された耳介。



The appearance
at the end of the 1st-stage operation
with bolster fixation sutures.

ボルスター縫合固定を行って手術終了。

小耳症手術による合併症
一過性の顔面神経麻痺　浅側頭動・静脈の血行不良による植皮の生着不良　感染、移植軟骨の露出　気胸　術後肺炎
縫合不全　ハゲ　床ずれ　その他
上記のような合併症が生じた場合は、症状に応じて対処致します。場合によっては再手術を行う可能性もあります。
