1st-stage operation for concha type microtia

耳甲介型小耳症の肋軟骨移植術


Preoperative appearance
　　 of concha type microtia.

耳甲介型小耳症の術前。


Determine the proper anatomical location
　 with transparent film template.

あらかじめ本人サイズの設計図を印刷した透明フイルムで
　耳介の正常な場所と大きさを設計する。


The outline for the 1st-stage operation.

肋軟骨移植術のデザイン。


Fabricated 3-dimensional costal cartilage frame
　　 [3-D frame] and paper template.

作成した3次元肋軟骨フレームと紙型。


Skin flaps formed,
　anterior skin flap of the lobule,
　posterior skin flap of the lobule,
　mastoid skin flap.
Almost remnant ear cartilage removed
　 except tragus, incisula inter tragica and anti-tragus.
Skin pocket created.

皮弁形成を行った。
　　耳垂前面皮弁、
　　耳垂後面皮弁、
　　乳突洞部皮弁、
耳珠、珠間切痕、および対珠を除く
　ほとんどの遺残耳介軟骨を摘出した。
皮下ポケットを作成した。


3-D frame is grafted
　　 under the skin pocket
　　　　　 and reconstructed auricle.

３次元肋軟骨フレームを皮下ポケットへ移植して再建した耳介。


At the end of the 1st-stage operation
　with Bolster fixation sutures.

ボルスター縫合固定を行って手術終了。

小耳症治療による合併症
一過性の顔面神経麻痺　浅側頭動・静脈の血行不良による植皮の生着不良　感染　移植軟骨の露出　気胸　術後肺炎　縫合不全　ハゲ　床ずれ　その他
　
上記のような合併症が生じた場合は、症状に応じて対処します。
場合によっては再手術を行う場合もあります。
