小耳甲介型小耳症（右耳）の肋軟骨移植術
Preoperative appearance
of small concha type microtia.
小耳甲介型小耳症の術前。
The outline for the 1st-stage operation.
肋軟骨移植術の手術デザイン。
Skin flaps formed,
anterior skin flap of the lobule,
posterior skin flap of the lobule,
mastoid skin flap,
tragus skin flap.
All remnant ear cartilage removed.
Skin pocket created.
皮弁形成を行った。
耳垂前面皮弁、
耳垂後面皮弁、
乳突洞部皮弁、
耳珠用皮弁。
すべての遺残耳介軟骨を摘出した。
皮下ポケットを作成した。
On the left side of the photo is a 3-dimensional costal cartilage frame,
and on the right side is a 3D model of an ear with a paper template.
今回、あらかじめ左の耳をスキャンし反転させた３D模型を使用した。
模型と紙型を比較すると、耳輪が紙型よりも外側に広がっていた。
このため、紙型も修正したものを用いて肋軟骨フレームを作成した。
左は作成した3次元肋軟骨フレーム、右は紙型を載せた3D模型。
3-D frame is grafted
under the skin pocket
and reconstructed auricle.
３次元肋軟骨フレームを
皮下ポケットへ移植して再建した耳介。
At the end of the 1st-stage operation
with Bolster fixation sutures.
ボルスター縫合固定を行って手術終了。

Complications after microtia surgery:
Pneumothorax (1 out of 2000 cases), alopecia, partial necrosis of skin flap, partial exposure of costal cartilage grafted, skin graft failure due to congenital poor blood circulation of superficial temporal artery and vein, etc.
In the event such complications arise, an additional surgery may be performed to address them.
◆今回の手術の術後合併症
■重篤な合併症
顔面神経や血管の走行に異常を認めることが多く、手術によって顔面神経麻痺や皮膚壊死を起こすことがあります。
また軟骨の採取部位では気胸・血胸・心タンポナーデの可能性があります。
■その他の合併症
①植皮の生着不良・壊死
②感染、移植軟骨の露出
③糸やワイヤーの露出
④傷跡が目立つ
⑤薄毛・脱毛
⑥長時間同じ体位による環軸椎亜脱臼｛第一頸椎（環椎）と第二頸椎（軸椎）｝
⑦長時間同じ体位による褥瘡
⑧その他、予測不可能な合併症
以上のような合併症が起こった場合は、再手術や処置を行う場合もありますが、不可逆的な状態もありえますのでご了承ください。
